Dhanvantari in Kashi

Sushruta Samhita, Sutrasthana 1.1

The story · English

Long ago, Dhanvantari, the greatest of the celestials, took birth on earth as Divodasa, king of Kashi. One day he sat at peace in his hermitage with holy Rishis gathered around him. Among them were Aupadhenava, Vaitarana, Aurabhra, Paushkalavata, Karavirya, Gopura-rakshita and Sushruta. Together they spoke to him. "Lord," they said, "it grieves us to see people fall to disease. Some suffer in the mind, some in the body. Some are wounded, and some are simply worn down by nature. They have family and friends around them, and still they cry out in pain as if they had no one in the world." The sages fell quiet for a moment and looked at the king. Then they told him plainly what they had come to ask.

"We ask you to open our minds to the truths of the eternal Ayurveda," they said. "Then we can do our duty in life and ease the suffering of everyone. Happiness in this life and the next is the gift of this science. That is why we have dared to come to you as your students." Dhanvantari answered, "You are all welcome to this hermitage, and every one of you is worthy to be taught." Then he began. Ayurveda was first a part of the Atharva Veda. Before human beings were made, Brahma set it down in a hundred thousand verses across a thousand chapters. But he saw how short human life is and how memory fails. So he wisely divided the whole science into eight branches.

Dhanvantari described each branch in turn. "The first is Salya-Tantram, surgery. It removes anything foreign from a wound, whether straw, stone, dust, iron, bone, splinters, nails, hair, clotted blood or hardened pus. It draws out a child who has died in the womb. It brings a safe birth when the baby lies the wrong way. It teaches how to hold and use surgical instruments, how to apply fire and caustic alkalis, and how to recognise and treat wounds. The second is Shalakya-Tantram. It treats illness in the openings above the collarbones, such as the ears, eyes, mouth and nose. The third is Kaya-Chikitsa. It deals with illness that seizes the whole body, like fever, dysentery, coughing blood, madness, hysteria, leprosy and unnatural discharges."

"The fourth is Bhuta-Vidya," Dhanvantari went on. "It teaches the chants and offerings that heal illness caused by spirits, demons, Gandharvas, Yakshas and Rakshas. The fifth is Kaumara-Bhritya, the care of children. It covers nursing infants, purifying a mother's milk when it is lacking, and curing the illnesses of early childhood. The sixth is Agada-Tantram. It treats the bites of snakes, spiders and venomous worms, and draws every kind of poison out of the body. The seventh is Rasayana-Tantram. It lengthens life, strengthens memory and the organs, and keeps a person vigorous into old age. The eighth is Vajeekarana-Tantram. It restores and strengthens a man's seed and the pleasures of youth. These are the eight branches. Now tell me, which shall I teach, and to whom?"

The disciples answered, "Teach all of us surgery, Lord. Let that be our main study." Dhanvantari said, "So be it." They spoke again. "We are all agreed that Sushruta will speak for us and ask the questions. The rest of us will listen closely to everything you tell him. That way you will not have to teach us one by one." The sage said, "So be it." Then he turned to his student. "Listen, Sushruta, my dear child. This science has two purposes. The first is to cure those who are sick. The second is to keep the healthy in health." He added that the word Ayurveda can mean the knowledge through which life exists, or the knowledge that helps a person live longer.

"Now hear me on surgery," said Dhanvantari. "It is the oldest branch of medicine. All four kinds of proof support it: what we see, what we reason, what we compare, and what scripture teaches. We know it is oldest because it heals wounds. There is a second reason too. The god Rudra once cut off the head of Yajna, the god of sacrifice. The gods went to the Ashvins and said, 'You twins, who will become the greatest of us, join Yajna's head back to his body.' The Ashvins replied, 'We will do as you ask.' Then the gods won over Indra, so that the Ashvins would receive a share of the sacrificial offerings. And the Ashvins rejoined Yajna's head to his body, just as the gods had asked."

"All agree that surgery is also the most important branch," Dhanvantari continued. "It holds everything the other branches hold, and with instruments it brings relief at once. So it is the highest of them. It is eternal and full of merit. It brings fame and opens the gates of heaven. It lengthens life and helps people fulfil their purpose and earn an honest living. Brahma first taught this science. Prajapati learned it from him. The Ashvins learned it from Prajapati and taught it to Indra, and Indra gave all of it to me. For the good of the world, I am ready to pass it on. I am Dhanvantari, the first god, who kept disease and old age away from the celestials. I have come down from heaven to teach surgery to people."

"In this science," Dhanvantari said, "a person, the Purusha, is the soul joined with the five elements: earth, water, fire, air and space. All treatment is given to the Purusha alone. The world has moving things and still things, and every one of them leans either hot or cool, because fire or water rules in it. Living beings arise in four ways: from warmth and moisture, from eggs, from the soil like plants, and from the womb. The Purusha is the greatest, since all the rest serve his needs. Whatever brings him pain is called disease. There are four kinds. Injury comes from outside. Bodily illness comes from bad food or drink, or disordered blood and humours. Anger, grief, fear, envy, greed and the like are mental. Hunger, thirst, old age, sleep and death are natural."

"Disease lives in the mind, the body, or both," said Dhanvantari. "Four things overcome it: cleansing, calming the disturbed humours, right diet, and right conduct. Food builds strength, colour and vitality. It has six tastes, found in the substances called Oshadhis. Some are still: trees that fruit without flowering, trees that flower and fruit, creeping vines, and plants like grain that die once their fruit ripens. Some move: animals born from the womb, from eggs, from moisture, and from decaying plants. From plants we take bark, leaves, flowers, fruit, roots, juice and resin. From animals, skin, nails, wool, blood, flesh, fat, marrow and bone. From the earth, gold, silver, gems, realgar, pearls and clay. Wind, sun, shade, moonlight, heat, cold, rain and the turning seasons are the work of time, and they raise or settle the humours."

"If an outside cause hurts the body," Dhanvantari said, "treat it as any bodily illness. If the mind is hurt, heal it with pleasing sounds, touch, sights, tastes and scents. So I have spoken briefly of the Purusha and all its parts, of Disease from the humours and blood, of Medicine with its qualities and powers, of Procedures such as surgery and oiling, and of the right Time for each. All this will be laid out fully in a hundred and twenty chapters across five sections: first principles, the causes of disease, the body, treatment, and poisons. Whatever remains will come in a closing supplement." And the chapter ends with this verse: whoever reads this eternal science, spoken by Brahma and spread by the king of Kashi, becomes known for goodness, is honoured by kings, and after death reaches the realm of Indra.
