The meeting of time, the objects of the senses and action, when too little, wrong or too much, and their right meeting, are to be known as the one cause of illness and of health. Illness is the doshas out of balance; their balance is freedom from illness. Illnesses are of two kinds: arising within, and coming from outside.
Vagbhata gathers every cause of sickness into one place: how a person meets time, the objects of the senses, and their own actions. Time means the turning seasons. Ayurveda explains elsewhere that it also covers the hour of day and the stage of life. The objects of the senses are what we see, hear, touch, taste and smell. Action is what we do with body, speech and mind. Each meeting can fail in three ways. It can be too little, like a winter that never turns cold. It can be wrong, like harsh sound or a strained posture. It can be too much, like hours of staring into glare. A right meeting brings health. The result shows in the doshas: vata, pitta and kapha, the three working forces that move, transform and hold the body together. Out of balance, they are illness. In balance, they are health. Illness then has two kinds: nija, rising from within, and agantu, arriving from outside. Elsewhere, Ayurveda gives examples of agantu such as injuries, falls, bites and poisons.
Think of rice on a stove. If the flame is too low, the grains stay hard. If it is too high, the rice scorches. If the pot sits off the flame, it cooks unevenly. Only a right flame, rightly placed, gives a good meal. The rice is the body. The flame is the meeting with time, the senses and action.
A family doctor would agree with much of the shape of this. Dose matters in nearly everything: too little or too much sleep, exercise, light or food each carries its own harm. Loud noise damages hearing, long hours of sitting strain the body, and some illnesses do rise and fall with the seasons. The body also keeps a daily clock, which fits the idea of time as a cause. Medicine likewise separates harm from outside, such as injury, from conditions that grow within. Two points differ. Today's medicine names many causes, such as germs, genes and environment, often working together, so it would not call any one thing the single cause. And vata, pitta and kapha have no measurable match in modern physiology; they belong to Ayurveda's own way of describing the body.
