Who Counts as Healthy

Sushruta Samhita, Sutrasthana 15.41

Meaning · English

One whose doshas are in balance, whose digestive fire is even, whose tissues and wastes do their work in due measure, and whose self, senses and mind are clear and glad, is called healthy.

The verse lists the marks a person must show before being called healthy. First, the doshas are in their proper measure, none swollen and none worn thin. These are vata, the force of movement; pitta, the force of heat and change; and kapha, the force that holds things together. Second, agni, the digestive fire, burns evenly. The classics describe uneven fire as sluggish, raging or erratic. Third, the dhatus, the tissues built from food, and the malas, the wastes the body sends out, are made and moved in due amount. Then the list reaches past the body. The self, the senses and the mind must be prasanna, a word meaning clear, settled and glad. The key idea is sama, evenness. In the text's reasoning, illness is any of these drifting out of measure, so health is their return to it. That sets a high bar. A person can be free of fever and still fall short of health in this sense, if digestion is unsteady or the mind is clouded and joyless.

Picture a pot of dal on the stove. The flame is steady, neither roaring nor guttering. Lentils and water are in the right proportion. Steam escapes under the lid as it should. And the cook beside it is calm enough to taste and notice. That is the verse's health: the flame is agni, the proportions are the doshas and tissues, the steam is waste leaving, and the cook is the clear mind.

Much of this sits comfortably with how a good family doctor thinks. Health is widely understood today as more than the absence of disease, and mental wellbeing is counted as part of it. Doctors still ask about appetite, digestion, bowel habits and mood, because changes in these are often early signs that something is off. The body does keep many things within steady ranges, such as temperature, fluids and blood sugar, which loosely echoes the idea of evenness. Where the match ends: the three doshas are not substances modern physiology can measure, and agni maps only loosely onto digestion and metabolism. Feeling well is also not a complete test. Some conditions, such as high blood pressure, give no signs at all, which is why routine checks matter even when every mark here seems present.
